ABSTRACT

Objective Toinvestigatethecharacteristicsofclinicalpathologyand MRIofintracranialsolitaryfibroustumor/hemangiopericytoma (SFT/HPC).Methods ThisstudyanalyzedtheMRIimages,pathologicalandclinicaldataof14SFT/HPCpatientsretrospectively. AllthecasesweresubjectedtoMRIplainscanandenhancementexamination.CharacteristicsofMRIofallcaseswerereviewedtogetherwith clinicopathologicchanges.Results AllSFT/HPClesionswerelocatedintheintracranialbutextra-cerebralspace.6werelocatedabovethe tentoriumofcerebellum,and2werelocatedbelowit.Lesionsof6patientswereacrossthetentoriumofcerebellumandspreadfrom supratentorialtosubtenorialspace.Amongalllesions,4wereroundinshape,10werelobulated,and3weresmallnodulesaroundthe edge.9ofthemexhibitednecrosisandcysticstructures.11lesionsshowedhypointensityand3casesshowedisointensityonT1WI.All thelesionswereheterogeneoushyperintensetyonT2WI,and5ofthemdisplayed"yin-yang"patternonT2WI.11casesexhibitededema.Signalof vascularvoidflow wasobservedin6cases.Thesolidpartsofthetumorsshowedsignificanthomogeneousenhancementon MRI. StrongpositiveSTAT6stainingwasobservedforthenuclearoftumorcells.Conclusion ItisdifficulttodifferentiateSFT/HPCfrom meningeoma.The"yin-yang"patternonT2WIisthecharacteristicofSFT/HPC.Inaddition,nuclearpositivestainingofSTAT6isalsospecific featureofSFT/HPCcell.

ABSTRACT

Objective To explore the application value of JCI standard and HIMSS 7 grade clinical closed loop system in pathological specimen handover. Methods The First Affiliated Hospital of Xiamen University adopted the traditional way to transfer pathological specimen in 2014, and improved the process under guidance of JCI standard in 2015. Then HIMSS 7 level clinical closed-loop system was applied to pathological handover on the basis of JCI standard guidance in 2017. Comparative analysis was made on the failure rate of the pathological specimen,the average handover time of pathological specimen and the timely rate of pathological frozen reports in the above 3 years. Results A comparison of the failure rate of pathological specimens found the following: That of 2015 was significantly lower than that of 2014 (P <0.05);and that of 2017(Jan. -Sept.) was significantly lower than that of 2015(P<0.01). A comparison of the handover timeliness of frozen digestive endoscopy specimens and a single frozen specimen handover duration found the following:That of 2015 was significantly shorter than that of 2014 (P< 0.01),and that of 2017 (Jan. - Sept.) was significantly shorter than that of 2015 (P < 0.01). A comparison of the timeliness of frozen specimen pathological reports found the following: that of 2015 was significantly better than that of 2014 (P< 0.05),and that of 2017 (Jan. -Sept.) was significantly better than that of 2015 (P< 0.05). Conclusions The guidance of JCI concept has reduced the failure rate of pathological specimen, shortened the average handover during, and improved the timeliness of pathological frozen specimen reports. Under the JCI standard guidance, the HIMSS 7 level clinical closed-loop system was applied to the pathological specimen handover process. This practice could significantly reduce the failure rate of pathological specimen and improve the pathological specimen handover efficiency. Furthermore,it is conducive to full-course tracking and dynamic management of such specimen.

ABSTRACT

Objective To explore the MSCT features,diagnostic values and the misdiagnosis causes of gastric schwannomas.Methods T he CT images,pathological and clinical data of 9 misdiagnosis cases of gastric schwannomas were analyzed retrospectively,which were later confirmed by the operation and pathology.The study examined the CT findings in the following aspects:the lesion location, size,contour,border,growth pattern,enhanced pattern,hemorrhage,necrosis,calcification,and the presence and absence of perirenal lymph nodes.Results The gastric schwannomas were ovoid or round,with well-defined boundaries.Tumors were found in the gastric body in 6 cases(4 cases in the large gastric curvature and 2 in the small gastric curvature),the gastric antrum in 2 cases and the gastric fundus in 1 case.The largest lesion diameters ranged from 2.5 to 7.4 cm,with an average diameter of 3.2 cm.Extracavitary growth was found in 2 cases,intracavitary growth in 2 cases and both intracavitary and extracavitary growth in 5 cases.The density of the tumor was low and evenly distributed,without cystic change,necrosis or calcification.2 cases were found to have ulcer formation. Another 2 cases were found to have enlarged lymph nodes,which were pathologically confirmed to be reactive hyperplasia.In the CT contrast enhancement,mild homogeneous enhancement was found in 4 cases in the arterial phase,but no obvious enhancement was found in the other 5 cases.In the venous phase,all the cases were found to have moderate homogeneous enhancement.4 cases were found to have further enhancement in the delay phase.Conclusion The gastric schwannomas appears to be homogeneous soft tissue mass on the MSCT,with clear boundaries and low-density and without hemorrhagic necrosis or cystic lesions.MSCT examination can help to locate and characterize gastric schwannomas,and to observe the relationship between the tumor and the surrounding tissue structure.

ABSTRACT

Objective To observe the effects of Taohechengqitang combined with danazol on CA125 and gonadal hormone in the treatment of patients with endometriosis.Methods 77 cases from July 2014 to January 2015 in department of obstetrics and gynecology of Daqing Longnan hospital were selected and divided into observation group of 40 cases and control group of 37 cases according to double-blind randomized.The observation group were treated with Taohechengqitang joint danazol treatment, and the control group were treated with danazol alone.The estradiol ( E2 ) , progestogen (P), cancer antigen 125 (CA125), thromboxane B2 (TXB2), endometrial cysts size and dysmenorrheal score pre-and post-treatment between two groups were compared.Results The contents of E2 and P post-treatment in observation group were lower than those in control group (P<0.05).The serum TXB2 and CA125 levels post-treatment in observation group were lower than those in control group (P<0.05).The endometrial cysts size and dysmenorrheal score post-treatment in observation group were lower than those in control group (P<0.05).The total efficacy in observation group was higher than that in control group (P<0.05).Conclusion Taohechengqitang combined with Danazol could reduce the CA125, E2 and P levels, relieve symptoms of dysmenorrhea and reduce the cyst volume, whose efficacy is exact.

ABSTRACT

<p><b>OBJECTIVE</b>To study the prevalence of ALK, ROS1 and RET fusion genes in non-small cell lung cancer (NSCLC), and its correlation with clinicopathologic features.</p><p><b>METHODS</b>Formalin-fixed and paraffin-embedded tissue sections from samples of 302 patients with NSCLC were screened for ALK, ROS1, RET fusions by real-time polymerase chain reaction (PCR). All of the cases were validated by Sanger DNA sequencing. The relationship between ALK, ROS1, RET fusion genes and clinicopathologic features were analyzed.</p><p><b>RESULTS</b>In the cohort of 302 NSCLC samples, 3.97% (12/302) were found to contain ALK fusion genes, including 3 cases with E13; A20 gene fusion, 3 cases with E6; A20 gene fusion and 3 cases with E20; A20 gene fusion. There was no statistically significant difference in patient's gender, age, smoking history and histologic type. Moreover, in the 302 NSCLC samples studied, 3.97% (12/302) were found to contain ROS1 fusion genes, with CD74-ROS1 fusion identified in 9 cases. There was no statistically significant difference in patients' gender, age, smoking history and histologic type. One non-smoking elderly female patient with pulmonary adenocarcinoma had RET gene fusion. None of the cases studied had concurrent ALK, ROS1 and RET mutations.</p><p><b>CONCLUSIONS</b>The ALK, ROS1 and RET fusion gene mutation rates in NSCLC are low, they represent some specific molecular subtypes of NSCLC. Genetic testing has significant meaning to guide clinical targeted therapy.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore the CT angiography features of the involved arterial branches of the spontaneous isolated superior mesentery artery dissection(SISMAD).</p><p><b>METHODS</b>Clinical and CT angiogram data of 6 patients with SISMAD confirmed by dual source energy CT angiography were analyzed retrospectively. The CT angiography features and prognosis of patients were investigated.</p><p><b>RESULTS</b>Average age of 6 patients was(45.83±5.60) years old. Ten branches of jejunoileal artery were involved in 4 patients. All the dissections of the superior mesenteric arteries(SMA) were extended from its proximal to its end. Intramural hematoma was found in all the involved branches, which showed eccentric target sign in the transverse section and sandwich sign in the longitudinal axial section. In 7 branches of 3 patients, the involved branches originated from the false lumina of the SMA. In one patient, the intramural hematoma of the jejunoileal arteries was found to be completely absorbed in a repeated CT angiography one year late.</p><p><b>CONCLUSIONS</b>For the SISMAD, the involvement of the branches of the SMA may be associated with the extension of the dissection of the SMA. The inflation of the false lumina of the branches is worse than that of the SMA. With the improvement of the dissection of the SMA, the intramural hematoma of the branches can be absorbed. The conservative treatment may be the first choice for SISMAD without emergency.</p>

ABSTRACT

Objective To estimate the value of diffusion weighted imaging with background body signal suppression(DWIBS)in tumors of intestinal tract and metastasis.Methods 15 cass with tumors of intestinal tract underwent MRI and DWIBS examinations.Four of them underwent CT scan.Results All primary tumors were showed by conventional MR sequences and DWIBS.DWIBS showed that 4 cases had the invasion of placenta percreta,5 cases had transcoelomic metastasis in abdominal cavity and 5 cases had lymphatic metastasis,while the conventional MR sequences showed the invasion of placenta percreta in 10 cases,the invasion of fat interspace in 8 cases,transcoelomic metastasis in abdominal cavity in 3 cases and lymphatic metastasis in 3 cases.Conclusion DWIBS is better than the conventional MR sequences and CT in detecting the primary tumor of intestinal tract,transcoelomic metastasis in abdominal cavity and lymphatic metastasis.However,the conventional MR sequences and CT is better than DWIBS in detecting the invasion of placenta percreta and fat interspace.

ABSTRACT

Objective To compare the advantages between CT and MRI examination and to evaluate the applied value of diffusion weighted imaging(DWI) in acute cerebral trauma.Methods 54 cases with cerebral trauma were undergone CT and MRI simultaneously,the advantages and disadvantages of both examination were analysed using self-comparative method. Results In showing subarachnoid hemorrhage, ultra-acute cerebral bleeding and bone fracture,CT was better than MRI,but cerebral contusion in 5 cases,laceration of white matter in 5 cases,acute white matter swelling in 2 cases and subdural haematoma in one case were misdiagnosed by CT.DWI was much better to show laceration of white matter and acute cerebral matter swelling than CT and conventional MRI.Conclusion In diagnosing acute cerebral bleeding and bone fracture, CT is better than MRI, for other diagnoses MRI better than CT. DWI should be applied to be a common scan method in diagnose of acute cerebral trauma.

ABSTRACT

Objective To study the findings of CT and MRI in tongue cancer.Methods 21 cases of tongue cancer were examined by CT or MRI,CT examined 7 cases,MRI examined 14 cases.Results MRI could display the tongue cancer in all the cases,but CT only display 5 of 7 cases of the tumour.The value of CT and MRI was same in displaying soft tissue direct invasion.MRI and contrast enhanced CT could display cervical lymph node metastases.CT was superior to MRI in the diagnosis of mandible invasion.Conclusion MRI is superior to CT in the display soft tissue of tongue cancer and the metastases.CT is optimal in detection of cortical bone invasion.

0.05).The values and ages were positive correlation before age 25(r=0.71~0.91,?0.05).Four main patterns of marrow distribution were identified in the proximal femur.Our data implied that there was progressive marrow fat distribution with age.The diaphyseal red marrow began to convert to yellow marrow after 5 years old and the conversion completed at age of 10.Foci of longer T 1 signals were found in the diaphysis in some of the subjects and the percentage increased with age.The femoral head and the greater trochanter showed yellow marrow signals after ossification.Conclusion Measurement of MRI signal intensity values could be used in evaluating the conversion of red to yellow marrow of the femur.Red and yellow marrow of femur may be distinguished by MRI.We should notice the different MRI feafure in different ages.

ABSTRACT

Objective To study the methods of delineating the whole length of bilateral facial nerve canals in one image. Methods High resolution computed tomography of the temporal bone was performed in 60 cases (120 ears) by Philips Mx8000 multislice spiral CT. Parameters: 120 kV, 200~250 mAs, Collimation: 0.5 mm, Pitch: 0.875, Scan time: 0.75 s/ring, Matrix: 512?512, Reformation interval: 0 1~0.2 mm, Reformation matrix: 1 024?1 024. Curved planar reformation(CPR)images were prepared along the facial nerve canal in the axial plane, and in the coronal and sagittal plane of multiplanar reconstruction(MPR). In the axial plane, a reference line was traced following the facial nerve canal from the internal auditory meatus, through the labyrinthine segment, the tympanic segment up to the second genu and mastoid segment. Another two protocols of curved reformatting were adopted: (a) a curved line was delineated along the facial nerve canal on the coronal reformatted image; (b) a curved line was drawn along the facial nerve canal on the sagittal reformatted image. The reference lines were carefully revised and moved exactly to the center of each segment of the facial nerve canal. For displaying bilateral facial nerve canals in one image, one reference line should be drawn along bilateral facial nerve canals. Results In 56 cases of 60 CPR, images in the axial plane, and coronal plane of MPR could show the unilateral or bilateral facial nerve canals clearly. The result of CPR of bilateral facial nerve canals in sagittal plane of MPR was unsatisfactory. The image on one side was often clear, but just part of it could be showed on the other. So the left and right facial nerve canals should be reformed separately. In 4 cases, CPR was unsatisfactory. In 1 of them the labyrinthine and tympanic segment had breaks because the patient′s head shook during the scanning. In 3 of them the facial nerve canals were showed unsatisfactorily because of the inexact position of head during the scanning. Conclusion The unilateral or bilateral facial nerve canals could be showed in one image by CPR. Combining the images of the high resolution CT in axial scan and MPR of the temporal bone, it would be helpful in the diagnosis of the lesions of facial nerve canal.

ABSTRACT

Objective To evaluate the value of routine head CT scan in diagnosing the fracture of orbital medial wall.Methods 40 cases doubted with the fracture of orbital medial wall were additionally scanned with orbital CT.Results Of 45 cases,35 cases were confirmed with fracture of orbital medial wall.The fracture signs on routine head CT included invagination of orbital medial wall,thick of medial rectus,increase in ethmoid sinus density and intra orbital gas.Conclusion Routine head CT scan can serve as diagnosing or screening the fracture of orbital medial wall.

ABSTRACT

Objective To assess the cerebral CT findings and clinical correlation in eclampsia.Methods Cranial CT scan was performed in 18 patients with eclampsia,3 cases underwent contrast-enhanced CT.Eight of them followed-up by CT scan.Results 14 cases had extensive low-density lesions represented cerebral edema involving the subcortical white matter in cerebrum with predominance in the occipital lobes,the frontal and parietal lobes also involved.4 cases appeared as small low-density foci in the brain,2 cases accompanied with multiple small intracranial hemorrhage.Conclusion CT scan is a good method in showing the cerebral lesions in patients with eclampsia,which can early find the lesions and can be used for follow-up.

ABSTRACT

Objective To evaluate the CT and MRI features of masticator space lesions.Methods All patients(43 cases) were examined with CT (28 cases) or MRI(10 cases),and 4 cases underwent both CT and MR examination.Results 9 cases of masticator space infection were found,which had characteristic appearances and patterns of widely spread on CT and MRI,and CT was better in detecting mandibular osteomyelitis.Primary tumors of the space included 6 cases of rhabdomyosarcomas,5 cases of angioma,4 cases of osteosarcomas.Most of them had the particular CT or MRI features.In 19 cases of metastasis of the space,most came from surrounding structures,especially from squamous cell carcinoma of the head and neck,and penetrating deeply into the muscles or ramus of masticator space.Conclusion The masticator space lesions(infection and most of tumors) are of characteristic features on CT and MRI.

ABSTRACT

Objective To measure the dimention and area of the cerebellar vermis and determine the effects of age and sex on the size of the cerebellar vermis in healthy adults.Methods We estimated the dimention and area of the cerebellar vermis in 360 healthy volunteers,20~87 years old.Results We found a significant reduction in the anterior-posterior dimention and the superior-inferior dimention after 60 years old.The anterior lobule area and the total area of the cerebellar vermis were larger in male than that in female.However,the analysis of age trends in the verian lobules revealed differential age related declines.Conclusion The dimention and area of cerebellar vermis reduced with aging after 60 years old.The area of the anterior vermis is larger in men than that in women.
